Job Description
Ward 31 at Darlington Memorial Hospital is a busy Trauma and Orthopaedic Unit caring for patients with trauma injuries and is also the designated area for patients admitted with head injuries.
The ward is a fast pace environment and can look after some very complex patients.
The discharge Coordinator role is to help support the nursing team in ensuring that all patient discharges and maintained to a high standard and also to assist with the complexities involved.
Main duties of the job
The main duties of this role will include:
Supporting nursing staff with all discharges, including very complex ones.
Liaising with all parts of the MDT such as Doctors, nurses, OT and physio.
Building good relationships and making referrals to social services and other support teams.
The role will involve a lot of autonomy and self organisation.
